How to Place an Order

Orders my be placed by fax or by email using the order form provided.

    Adobe Reader is required

1. Download the order form as a .pdf file.
2. Fill in the Contact Information and Payment Method Sections on page 1, including the purchase order number only if not paying by credit card.
3. Fill in the Quantity fields on page 2.
4. Fill in the appropriate shipping charge on page 2. The higher shipping charge may apply to orders greater than US $1,500. Please enquire.
5. The total amount, including the insurance charge, should be automatically calculated.
6. Either:
   1. Print out this form and fax it to (416) 490-1535 or 1-877-481-1117 (toll free in Canada and the US).
   2. Save this form and email it to linshincanada@hotmail.com (requires the full version of Adobe Acrobat).
7. Order will be sent within 15 business days upon receipt of completed Product Order Form.
8. Payment may be made by cheque, by e-transfer, or by credit card.

Alternatively, the client's own purchase order may be faxed to (416) 490-1535 or 1-877-481-1117 (toll free in Canada and the US). Order will be sent within 15 business days upon receipt of client's own purchase order.

Payment

Payment can be made by cheque in U.S. dollars drawing on a bank in the United States or Canada, by direct e-transfer, or by credit card (Visa, MasterCard, or American Express).
